DVD to Cloud Streaming Exchange

This ticket form is for customers wishing to switch from a DVD set to a renewable 12-month streaming subscription who received their DVD set more than 30 days ago. If it has been thirty days or fewer since you received your DVDs, please contact us by email or phone instead.

Submit this order form before mailing your DVDs. Print the order confirmation email and include it in your package. Mail to: Clear Water Press, Attn: DVD Exchange, P.O. Box 62, Olathe, KS 66051.

We recommend paying for tracking or delivery confirmation, but the carrier is your choice. Please avoid Media Mail as materials often arrive damaged.

We will set up your Cloud subscription upon receipt of your returned DVDs. If you need access to your Cloud subscription sooner, please email your tracking number to us at info@clearwaterpress.com.
